What are they?
Claim: A formal request for your insurance company to provide coverage for your medical expenses.
EOB: Shows how much the insurance paid, your responsibility or what information is needed to complete your claim.
What do they show?
Claim: Information about your medical treatment and the related charges
EOB: Shows how much your insurance plan covered for those charges
Who sends them?
Claim: Most times they are sent by in- network medical provider. There may be instances when you will need to submit a claim on your own.
